To every patient and caregiver,
CareDenza was born out of necessity.
I've been a doctor for six years. I've cared for thousands of patients — and no matter how much we prepare, there's one moment every patient, doctor, and caregiver quietly dreads: discharge.
In the hospital, we're with you every step of the way — managing your medications, monitoring your vitals, making sure you understand what comes next. The moment you walk out the door, all of that falls on the patient and their loved ones. And that's a lot to carry.
Patients come back to the hospital for many reasons. Infections. Heart failure. A complication nobody caught in time. But the hardest ones to accept are the preventable ones — the patient who didn't understand their discharge instructions, the medication taken at the wrong time, data that wasn't tracked, follow-up appointments that slipped through the cracks.
I see it every day. And after years of watching it happen, I finally asked myself: what would someone actually need to make it home safely and stay there?
So we built the app we wished our own families had.
As a patient, you photograph the discharge form or prescription pill bottles. The app reads every line, turns it into a plain medication list, and explains each medication in words you can actually use. The dose schedule organizes itself. Reminders arrive quietly. And your daughter two states away can open the app and see exactly how the day is going.
CareDenza is built for both the patient and the caregiver — most health apps pick one or the other, but care after discharge is something a family tackles together.
We wanted something that felt serious without feeling clinical. Calm colors. Generous spacing. Big text and big buttons.
